These products are not typically covered by standard health care insurance for these purposes. This is a test that provides information which may aid in the support of nutritional well being. It is not intended to diagnose, treat, or cure disease. Please note that the only invoice available is the one you will receive with your order. We do not provide invoices with CPT codes for insurance purposes.
Sorry, this test is not available to residents of New York and Maryland. Any orders originating from the state of New York or Maryland will automatically be canceled. Any samples originating from the state of New York or Maryland will be discarded.

Samples being returned within in US will have prepaid return postage enclosed.
Samples being returned internationally (from outside the US) will not have prepaid return postage, it will be your responsibility to cover the cost of returning the sample to the lab.
